If you are an existing patient and would like to make an appointment, please refer to the TheraNest Client Portal to view and request available times. 
If you are a new patient, please fill out the intake forms on the become a patient page and follow the instructions on the intake form.

Our office is open Monday through Thursday 9 a.m. to 5:30 p.m. and Friday 9 a.m. to 3 p.m. Most appointments are available during the morning and afternoon hours. There are select early evening appointments, however, these appointments book quickly. Should an emergency occur after business hours, please call Crisis Services at (716) 834-3131. Please note, if you are in the office, masks are required regardless of vaccination status, to promote the safety of others. 

We accept cash, check, money order, or card (Healthcare FSAs are accepted as well). Please note, cards will incur ~3.5% fee. Your bank statement will reflect Danielle Cook Mental Health Counseling. A card is required to be on file. All checks and money orders must be made payable to Danielle Cook (Please do not make them out to CPH). Please be advised, we do not accept partial payments. If you are unable to pay your full appointment fee by the time of your session you will not see a therapist. If we have not received check, cash, or money order payment by the time of your session, your card will be charged before the session can begin.

We take pride in the time and care we give to each patient. Insurance plans often limit the time a practitioner can spend with a patient and sometimes the kind of care; therefore, our providers currently do not participate in network plans. However,  our foundation is able to offer some financial assistance. If you would like to be placed on the sliding scale, please provide your entire household's most recent W2's or last 4 paystubs with the intake packet. Additionally, you can check with your insurance provider to verify if they have a reimbursement policy. Some insurance companies are able to reimburse up to 100% of the cost of a telesession visit.

We have a small parking lot behind Catholic Charities. Parking is limited. If there are no available spaces in that lot street parking is available. Please allow extra time to find parking.

Yes, however, everyone's journey is unique. To find out how to move your transition forward, please make an intake appointment to speak with a clinician.

Yes! We have the credentials to write letters for transgender surgery, however, everyone's journey is unique. We typically only write recommendation letters for current patients in good standing.  To find out how to move your transition forward, please call (716) 322-6394.

Yes! We are always happy to see past patients! If it has been over one year since your last appointment, your information will need to be updated, please fill out the intake packet on the become a patient page and send it back to us.

Yes! CPH's mission is to promote the best care possible for children and adolescents who have a wide range of endocrine conditions and sexual developmental concerns. CPH, along with a partnership from the Children’s Growth Foundation (CGF), follows a model of care for these children and their families based on a team approach which includes the family, mental health professionals, and medical specialists.
